About The Position

Ambulatory Patient Care Coordinator is an essential part of the healthcare team, ensuring smooth operations and a high level of patient satisfaction in outpatient or ambulatory care settings. Requirements

• High School Diploma Required
• Up to 1 year
• Three (3) to six (6) months on-the-job training as required to attain proficiency of assigned duties and responsibilities if not certified. Required
• Communication Skills: Strong verbal and written communication abilities to effectively interact with patients, families, and healthcare professionals.
• Organizational Skills: Excellent time-management and organizational capabilities to handle multiple tasks and prioritize effectively.
• Problem-Solving Skills: Ability to handle patient concerns and resolve scheduling conflicts with professionalism and efficiency.
• Empathy: A patient-centered approach, demonstrating understanding and compassion for individuals receiving care.
• Teamwork: A collaborative mindset to work effectively as part of a multidisciplinary healthcare team.
• Attention to Detail: Up to date record-keeping and detail-oriented work to ensure accuracy in patient care coordination.
• Technical Proficiency: Proficiency in using EMR systems and office software.

Responsibilities

• Patient Scheduling and Coordination: Schedule and manage patient appointments, follow-ups, and other procedures to optimize clinic flow and reduce wait times.
• Patient Communication: Serve as the liaison between patients and healthcare providers, addressing inquiries and providing detailed information about treatments and services.
• Care Planning: Assist in developing and implementing patient care plans, ensuring that all necessary services and treatments are coordinated effectively.
• Documentation and Records Management: Maintain accurate patient records, including medical histories, test results, and treatment plans, in compliance with healthcare regulations.
• Insurance and Authorization: Work with insurance providers to verify coverage and obtain necessary authorizations for procedures and treatments.
• Collaboration with Medical Staff: Partner with doctors, nurses, and other healthcare professionals to ensure seamless delivery of care and address any patient concerns.
• Quality Improvement: Identify opportunities to improve patient care processes and assist in implementing solutions that enhance efficiency and satisfaction.
