About The Position

The Oncology Nurse Navigator will work collaboratively with the Community Hem/Onc teams when a patient is referred to the department and throughout the cancer continuum. An oncology nurse navigator (ONN) is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The ONN will work with the ONS core competencies as the guide. The ONN competencies to be performed: (a) coordination of care, (b) communication to patient, families, and members of the healthcare team, (c) patient and family education (d) professional role.

Requirements

  • Two years of Adult Hematology Oncology nursing experience within the past five years.
  • Current OCN (Oncology Certified Nurse) certification or complete 36 cancer-related continuing education nursing contact hours each accreditation cycle, per Commission on Cancer.
  • Baccalaureate Degree in Nursing from a program accredited by CCNE, ACEN, or CNEA 30 days before start date for BSN graduates.
  • Associate Degree in Nursing from an accredited program 30 days before the start date for ADN graduates.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire.
  • Current, unencumbered Oregon State Registered Nurse License.
  • Washington RN license within 6 months of hire.
  • BLS at time of hire issued by the American Heart Association (AHA).
  • Must be able to perform the essential functions of the position with or without accommodation.

Responsibilities

  • Work collaboratively with the Community Hem/Onc teams when a patient is referred to the department and throughout the cancer continuum.
  • Provide individual assistance to patients, families, and caregivers to help overcome healthcare system barriers.
  • Facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.
  • Ensure that all baseline clinical information is gathered prior to the patients visit.
  • Collaborate with Team Coordinators and Managed Care Coordinators to gather appropriate business-related information prior to the visit.
  • Assign tasks to team members after assessing patient needs, staff skills, and resources.
  • Expedite patient flow during clinical practice sessions.
  • Ensure that Provider orders resulting from the new patient visit are recorded and executed.
  • Track post-visit flow information to ensure plans of care are executed in a timely fashion.
  • Troubleshoot any problems that patients and their families may encounter in preparing for or during their hospital stay.
