About the position

The Oncology Patient Navigator provides comprehensive support and guidance to cancer patients and their families throughout the cancer journey. This role serves as a central point of contact, helping patients navigate the complexities of cancer care, from diagnosis to treatment and beyond. The Navigator works collaboratively with a multidisciplinary team to ensure patients receive timely, coordinated, and patient-centered care.

Responsibilities

  • Facilitates the patient's timely access to medical and psychosocial care across the continuum from screening, diagnosis, treatment, rehabilitation, survivorship and/or end of life.
  • Coordinates a seamless model of access and care benefiting patients, family members, and participating physicians throughout the GSH system including both primary and secondary service areas.
  • Utilizes a collaborative, interdisciplinary approach to maximize utilization of resources and minimize duplication of services.
  • Maintains effective and timely communication with all members of the patient's healthcare team.
  • Assists in rooming of patients, as needed.
  • Ensure all labs are placed and drawn as ordered.
  • Develop therapy plans.
  • De-access ports.
  • Obtain specialty lab samples in clinic and ship to specialty lab.
  • Provide education to patients regarding treatment, recommendations, upcoming procedures, and follow-up appointments, including chemotherapy regimens.
  • Triage incoming referrals, timely scheduling of patients, and clinical telephone calls.
  • Monitor patient lab results and facilitate any additional treatments needed during chemotherapy course.
  • Facilitates interdisciplinary care planning meetings as appropriate.
  • Acts as a patient advocate, ensuring their needs and preferences are heard and respected.
  • Identifies and addresses barriers to care.

Requirements

  • RN, graduated from an accredited school of nursing; obtain and maintain applicable certification within one year of becoming eligible.
  • Practical work experience in oncology with preferred experience in outpatient oncology and working with financial aspects of cancer care (CMS, third party payors, etc.).
