The Ambulatory Nurse Care Manager is responsible for coordinating care for an identified population of at-risk and emerging risk patients followed in the primary care clinics. The Ambulatory Care Manager will follow the philosophies of case management to provide care coordination services in order to make sure that patients receive well-coordinated care along the health care continuum and promote quality care through appropriate, cost effective interventions. The Ambulatory Care Manager will promote preventative care and address condition specific needs of the patient population. This position requires problem-solving, communication with patients, providers and ancillary staff and clinical expertise to address the patient's barriers to care.
