About The Position

The Ambulatory Nurse Care Manager is responsible for coordinating care for an identified population of at-risk and emerging risk patients followed in the primary care clinics. The Ambulatory Care Manager will follow the philosophies of case management to provide care coordination services in order to make sure that patients receive well-coordinated care along the health care continuum and promote quality care through appropriate, cost effective interventions. The Ambulatory Care Manager will promote preventative care and address condition specific needs of the patient population. This position requires problem-solving, communication with patients, providers and ancillary staff and clinical expertise to address the patient's barriers to care.

Requirements

• Bachelor's Degree BSN
• 3-5 years experience
• Nursing experience caring for patients with chronic disease or medical complexity.
• One of the following required upon hire: Licensed RN - MO, Registered Nurse Multistate License Missouri; Licensed RN - Kansas, Registered Nurse Multistate License Kansas; RN Compact license for all states besides KS and MO
• Employees must obtain KS or MO Multistate License within 90 days of hire
• One of the following required within 3 years of hire: Certified Case Manager (CCM); American Case Management (ACM); Nursing Case Management (CMGT-BC); Care Coordination and Transition Management Certification (CCCTM)

Responsibilities

• Provides care management and coordination of services for patients utilizing the case management process.
• Maintains timely documentation in the EHR to enhance communication and collaboration across the healthcare system.
• Supports the National Committee for Quality Assurance (NCQA) standards for Case Management.
• Meets the educational needs of the patient and family and advocates for their needs.
