The Nurse Navigator supports patients with neuroimmunology and neuromuscular diagnoses throughout their care journey. This role coordinates complex treatment plans, facilitates timely access to services, and ensures patient education and advocacy across settings including outpatient, infusion, and community care. Requirements

  • Registered Nurse (RN) with current NYS licensure
  • Ability to manage complex care needs and work collaboratively across teams

Responsibilities

  • Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families
  • Onboard new patients starting DMTs or infusion therapies, including consent and education
  • Coordinate care for patients receiving treatment at home or outside infusion centers
  • Track labs, MRIs, prior authorizations, and medication logistics
  • Collaborate with Infusion Navigator, providers, schedulers, and social work to remove barriers to care
  • Support biosimilar transitions and medication teaching
