Join UChicago Medicine, as an Ambulatory Medication Access Coordinator on the Meds2Beds transitions of care team in the Pharmacy Department. In this role, you will be responsible for various tasks related to prescription benefits and medication access for patients discharging home from the hospital. You will gain exposure to inpatient care, outpatient pharmacy, and transitions of care. The Ambulatory Medication Access Coordinator plays a key role on our Meds2Beds team in order to provide optimal patient care at the time of discharge. UChicago Medicine is dedicated to providing patients with the personal service necessary to ensure that they achieve the most benefit from their medication therapy. Eligible for a Sign-On Bonus up to $3,000 In this role, the Ambulatory Medication Access Coordinator (AMAC) performs necessary functions to ensure medication access for patients discharging home from the hospital. Other areas of responsibility include pre-screening patients for pharmacy prescription insurance, enrollment of patients into the Meds2Beds service, managing cost and payment associated with take-home medications, identifying opportunities for pharmacist intervention on prescriptions, completion of necessary paperwork for prior authorizations, performing prescription insurance test claims via consultation from providers, and communication with the multidisciplinary care team.
