The Anticoagulation Clinic nurse is a professional who utilizes excellent communication and customer service skills in assessing, counseling, and educating patients in lifestyle modification as it relates to anticoagulation therapy. The nurse is responsible for monitoring the patient's INR and works within the confines of physician-developed order sets in formulating a plan of care for the patient. Collects statistics for performance improvement and financial tracking purposes as directed. The delivery of professional nursing care at Stormont Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.
