The Ambulatory Complex Care Manager promotes the achievement of optimal clinical and resource outcomes by assessing, planning, implementing, coordinating, and monitoring interventions and develops a comprehensive plan of care for the patients. Performs ongoing communication and works to promote quality of care through collaboration with the Interdisciplinary Care Team. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The Ambulatory Complex Care Manager assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with population management and transition plans. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Works with the Care Management Division leadership team to ensure alignment with the strategic goals to meet OSF Key Results.

  • Assess, plan, implement, coordinate, and monitor interventions for patients.
  • Develop a comprehensive plan of care for patients.
  • Promote quality of care through collaboration with the Interdisciplinary Care Team.
  • Coordinate health care services on behalf of the patient.
  • Address complex psychosocial and behavioral health issues.
  • Work with the Care Management Division leadership team to align with strategic goals.
  • Bachelor’s degree in Social Work (BSW).
  • Social Work licensure compliant with Illinois and/or Michigan state requirements.
  • 3 years of experience caring for patients.
  • Demonstrated broad knowledge of chronic illness and medical/community resources.
  • Working knowledge of a clinical EMR system and Microsoft Office applications.
  • Ability to use independent judgment and make decisions with little supervision.
  • Excellent interpersonal skills to maintain cooperative working relationships.
  • Master’s degree in Social Work (MSW).
  • Experience with chronic, complex illnesses adult and/or pediatric care management.
