About The Position

The Ambulatory Clinical Pharmacist (ACP) plays a vital role in delivering integrated, patient-centered care within designated clinics. This position focuses on optimizing therapeutic outcomes for individuals with chronic conditions by fostering strong relationships with patients and providers, enhancing medication adherence, and implementing evidence-based treatment strategies. The ACP works on-site and collaborates closely with both hybrid and onsite teams to maximize clinical services and continuity of patient care.

Requirements

  • Doctor of Pharmacy (PharmD) and active Texas and Louisiana pharmacist license required.
  • Minimum 3 years as a clinical pharmacist
  • Strong clinical judgment, communication, and process management
  • In-depth knowledge of pharmacology, FDA regulations, and chronic disease management.
  • Proven ability to ensure legal compliance and deliver high-quality, detail-oriented care
  • Organized, dependable, empathetic, and collaborative with a strong focus on integrity and patient-centered care.
  • High energy, team-oriented mindset, and ability to handle sensitive data with discretion and confidentiality.

Nice To Haves

  • PGY1/PGY2 residency preferred.

Responsibilities

  • Identify, resolve, and prevent medication-related problems to ensure safe and effective pharmacotherapy.
  • Conduct comprehensive assessments for chronic disease management under collaborative practice agreements or clinic protocols.
  • Stratify patients based on clinical risk to prioritize interventions and tailor care plans accordingly.
  • Provide individualized counseling on medication use, lifestyle modifications, and disease state education.
  • Address barriers to adherence, including cost, access, and health literacy, through targeted interventions.
  • Monitor and evaluate medication regimens to ensure clinical criteria are met, including therapeutic goals, adverse effects, and drug interactions.
  • Work on-site in close coordination with interdisciplinary teams, including both in-person and hybrid care models.
  • Ensure all clinical criteria are in clinical compliance with guidance from FDA, clinical guidelines, and appropriate evidence-based literature.
  • Willing to do whatever is necessary for the betterment of the organization.
  • Protects organization's value by keeping information confidential.
  • Other duties as assigned by Clinical Leadership or Manager of Clinical Services.
