The Ambulatory Clinical Pharmacist provides comprehensive pharmaceutical care for patients in the ambulatory setting. This consists of ensuring drug therapy that results in safe, effective, and cost-conscious use of pharmaceuticals. In addition, the pharmacist ensures patients have appropriate access to prescribed medications and are provided with medication education as well as adherence tools when needed/appropriate. Candidates must demonstrate excellent interdisciplinary communication and exhibit strong educational and precepting abilities. This pharmacist will work in the pulmonary and rheumatology clinics alongside the multidisciplinary team to serve patients with pulmonary (interstitial lung diseases, eosinophilic disorders / vasculitis, non-cystic fibrosis bronchiectasis) and various rheumatologic disease states on complex medication(s). They will be responsible for initial clinical evaluation, patient education and ongoing monitoring of all patients with pulmonary / rheumatologic diseases on specialty medications.
