Organization: ABSMC-Alta Bates Herrick Campus Position Overview: Delivers pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost efficient medication therapy. Exercises sound judgment, and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. This position requires flexibility to orient and work at provider clinics and specialty pharmacies. Clinical knowledge and therapy management for Chronic Inflammatory disease states/HIV/Oncology. Maintain excellent written and verbal communication skills and strong critical thinking skills. Excellent organizational skills, and ability to prioritize/follow-up. Flexible; able to cope with rapid change; comfortable with an ambiguous environment. Ability to troubleshoot problems efficiently and effectively. Ability to provide comprehensive medication management including initial assessments, toxicity evaluations/symptom management, and reassessments. Capability to set priorities amongst a series of competing clinical and operational priorities and to effectively coordinate and direct the work of others to meet specified deadlines. Proficient computer experience and accurate data entry skills in MS Word and Excel.
