About The Position

This is a part time (24 hours/week) Pharmacist position. This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes.

Requirements

• Current Indiana License or eligibility for Indiana licensure as a pharmacist
• Successful completion of an ASHP accredited PGY1 and/or PGY2 pharmacy residency may be required

Responsibilities

• Ensure safety and effectiveness of the medication use process
• Process and dispense medications per order or prescription
• Monitor medication therapy
• Ensure regulatory compliance
• Maintain oversight over pharmacist extenders
• Maintain the security of medications
• Protect the confidentiality of patient information
• Provide drug information support for medical staff
• Ensure continuity of care
• Identify potential challenges in common situations
