About The Position

The position is for a Registered Nurse in the Wake Population Health department, focusing on the Navigator Program. The role involves ongoing evaluation and quality improvement of the program, ensuring quality patient care, and implementing outreach efforts to maintain relationships with community agencies. The nurse will collect and maintain program data, deliver reports on outcomes, and participate in professional development activities.

Requirements

• Graduation from an accredited School of Nursing.
• BSN required with two years' experience as a Registered Nurse.
• Current license as a Registered Nurse (RN) in the applicable state.
• BLS certification required.
• Case management experience preferred.
• Bilingual preference with fluency in Spanish preferred.

Responsibilities

• Evaluate and improve the Navigator Program and track all activities as per departmental standards.
• Ensure quality patient care by involving patients and families in identifying needs and creating care plans.
• Identify barriers to patient care management and seek strategies to remove them.
• Implement outreach efforts to establish and maintain relationships with community agencies.
• Collect and maintain accurate program data and documentation.
• Deliver reports demonstrating outcomes and performance improvement activities.
• Participate in shared governance and take personal responsibility for professional growth.
• Provide age/developmentally appropriate patient care.
