About The Position

Under the supervision of the Director, Population Health/Case Management, the Ambulatory Care Manager Navigator – RN is responsible for assessing and identifying psychosocial needs for the Mount Nittany Physician Group patient population. Manages high risk/high cost patients by creating a care plan, providing education, psychosocial support, and connecting patients to community resources. Effectively communicates plan to Providers. Continues to assess patient’s progress on care goals with regular and ongoing communication. Develops and maintains relationships with community based organizations. Requirements

  • Master of Science in Nursing with a minimum of two years’ experience in a health care setting. OR Bachelor of Science in Nursing with a minimum of four years current experience in a health care setting. An Associate Degree or Diploma RN with a minimum of eight years clinical experience and four years care manager or case manager experience will be considered.
  • Ability to effectively assess and communicate with patients, their families, providers, and outside agencies.
  • Ability to independently work and problem solve based on patient’s needs.
  • Strong interpersonal skills; ability to communicate effectively with all levels of management and staff across the System.
  • Exceptional customer service and demonstrated effective interaction with physicians, patients, and staff.
  • Oral and written communication skills; Strong diplomacy and collaboration skills.
  • Sound decision-making skills.
  • Travel to office locations is expected.
  • If incumbent is a Registered Nurse, must be licensed as a Registered Nurse in the state of Pennsylvania.

Nice To Haves

  • Preference is given to previous case management experience.
  • Case Management Certification preferred.
