Are you living your life's work? LOCATION: East Petersburg, PA HOURS: Full Time (40 hours per week). Monday-Friday. No weekends or holidays. Our Care Management team is seeking an experienced Social Worker (MSW) to support AmeriHealth members experiencing housing insecurity and those currently experiencing homelessness. The Ambulatory Care Manager should be self driven and adaptable, working collaboratively with various departments, external healthcare systems, and community-based organizations. This is a fully grand funded position through AmeriHealth Cartias Grand Funds. Summary: Will provide support to physicians, clinical staff and patients who fall under the auspices of the practice Medical Home Program. Uses an explicit process to identify patients at risk for poor outcomes, and who coordinates care and support, both within and outside of Community Health Services.

Requirements

  • Social Worker with a Master's degree in Social Work (MSW).
  • Three (3) years of social work experience.
  • Excellent verbal and written communication skills.
  • Excellent customer service skills.
  • Proven informal leadership skills.
  • Ability to work independently and set priorities to coordinate care plans efficiently.
  • Ability to work effectively in a fast-paced team environment.
  • Highly organized and detail-oriented with the ability to perform multiple tasks simultaneously.
  • Effective behavioral and educational strategies, including motivational interviewing, teach-back method, and self-management support.
  • Must have a valid driver's license and daily access to an insured vehicle.
  • Knowledge of Utilization review or managed care.
  • Licensure with Commission for Case Managers (CCM), or expected to obtain within two (2) years of employment.
