Our Care Management team is seeking an experienced Social Worker (MSW) to support AmeriHealth members experiencing housing insecurity and those currently experiencing homelessness. The Ambulatory Care Manager should be self driven and adaptable, working collaboratively with various departments, external healthcare systems, and community-based organizations. This is a fully grand funded position through AmeriHealth Cartias Grand Funds. Summary: Will provide support to physicians, clinical staff and patients who fall under the auspices of the practice Medical Home Program. Uses an explicit process to identify patients at risk for poor outcomes, and who coordinates care and support, both within and outside of Community Health Services.
