Ambulatory Care Lead Nurse
About The Position
Under indirect/direct supervision, performs in a Nurse Lead role within the scope of the RN practice role, to coordinate and provides comprehensive patient care and associated services within a defined specialty practice. Works with other clinic staff and oversees operational clinical workflow, assesses patient/family needs; coordinates care and associated procedures or services for complex patients; provides education to patient/family; provides telephone advice to patient/family, based upon protocol; functions as an advocate and resource for patient/family & health care team; and facilitates communication within health care team. Mentors other clinical staff & delegates tasks to appropriate staff within the practice, collaborates with other health care providers, schools and/or community services to meet patient/family needs. Collaborates with Operation Managers in achieving the clinic specialty goals. Connecticut Children’s is the only health system in Connecticut that is 100% dedicated to children.
