Ambulatory Care Clinical Pharmacist - Internal Medicine
About The Position
The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record. UVA Health is recruiting two Ambulatory Care Clinical Pharmacists to support Internal Medicine specialty care in high‑volume ambulatory clinics. In this embedded role, pharmacists partner closely with internal medicine physicians and care teams to deliver comprehensive, patient‑centered care.
Requirements
- Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.
- PGY-1 residency required or equivalent clinical experience.
- Commonwealth of Virginia State Board of Pharmacy License required.
- American Heart Association (AHA) Health Care Provider BLS certification required.
Nice To Haves
- Board Certification or equivalent certification specific to clinical area highly encouraged.
Responsibilities
- Collaborate with Internal Medicine physicians as an integrated member of the care team
- Provide direct patient care and comprehensive medication management
- Optimize therapy for complex and chronic disease states
- Support specialty internal medicine workflows and population health initiatives
- Educate patients to improve adherence, safety, and outcomes
- Work within a Collaborative Practice Agreement to manage pharmacotherapy plans
