The Ambulatory Cardiac Clinical Dietitian functions as a key member of the interdisciplinary care team, providing nutrition services to patients in the cardiac, cardiac rehabilitation, cardiothoracic surgery, and general surgery ambulatory clinics through a hybrid work model that includes both on-site and remote care. This role involves completing comprehensive, individualized nutrition assessments; developing and implementing personalized nutrition care plans; and conducting ongoing follow-up to evaluate patient progress. The dietitian regularly reviews clinical status and outcomes, adjusting nutrition interventions as needed to support recovery, optimize cardiovascular health, and promote surgical outcomes.
