Reporting Directly to our Founder, Dr. Jennifer Pilcher, will play a crucial role in enhancing the quality of life for our clients by ensuring that comprehensive and compassionate care is delivered. And this is how they do it… Conduct comprehensive in-home assessments and draft written summaries of findings to provide to clients and/or families Develop goal oriented written care plans within a week of the initial meeting with client. Implement various components of the care plan and update care plan on a regular basis, as appropriate Negotiate family dynamics to implement care plan. Work within existing family systems to ensure client's needs are met and families have the tools needed to communicate and work towards realistic goals Communicate regularly with family members and appropriate professionals Cultivate and maintain extensive knowledge of community resources available to those with disabilities Collaborate with professionals and advocate for clients’ needs Coordinate client’s medical providers and medications, collaborate with providers to ensure good communication and consistent disease management Our clinical team consists of social workers, nurses, occupational and physical therapists, Expressive Arts Therapists and other professionals, all dedicated to our clients providing them with resources, therapies, direction and guidance when they need it most. Requirements

  • RN, LCSW, LSW, LICSW, MSW or related discipline. Experience with the aging population, resources and housing is essential.
  • Masters Degree in Social Work, Gerontology, Psychology, Nursing, or related field
  • 5 years or more experience in the field of disability or mental health setting
  • Demonstrated clinical knowledge of intellectual and developmental disabilities, traumatic and acquired brain injuries, mental illness, and Autism spectrum disorders
  • Experience working with complicated family systems and family dynamics
  • Experience negotiating services and systems
  • Computer skills and knowledge
  • Ability to write clear and professional notes
  • On-call availability team rotation
  • Possess qualifications for certification
  • Ability to function effectively in a group setting/team player
  • Driver's license in good standing
  • Commitment to integrity, excellence, and the highest standards of ethics

Nice To Haves

  • Knowledge of local resources (preferred)
  • Experience working with mental health issues and coordinating services for clients with mental illness (preferred)
  • Advanced degree in social work, nursing, PT, OT, or other related specialty
  • Experience with intellectual and developmental disabilities, mental health issues and autism spectrum disorders.
