About The Position

Position: · This is professional work developing care plans and managing services for older adults. An employee in this job assesses needs and manages social services for older adults and their families to improve their quality of life. · Work involves evaluating the social contacts and supports, daily living activities, cognitive functioning, physical health, and economic and other resources of older adults to develop plans for their care. · Work includes coordinating plans with other agencies to refer older adults for additional services and assisting older adults in utilizing available services to meet their needs. · Work may also include conducting more thorough assessments of an individual’s medical, social, and psychological well-being, as well as their living situation, including the physical condition of their housing unit, to ensure they receive the best care. · Work is performed independently and reviewed by a professional or administrative supervisor through individual and group conferences and an evaluation of case records and reports

Requirements

  • Must meet PA residency requirement.
  • Employment preference may be given to residents of Northumberland County.
  • Must be able to travel throughout Northumberland County and to State-mandated trainings in areas that may or may not be accessible by public transportation; travel expenses will be reimbursed.
  • Must be able to perform essential job functions.
  • Valid PA Driver’s license.
  • Six months as an Aging Care Manager 1 or a County Caseworker 1
  • Successful completion of the County Social Casework Intern program
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field
  • An equivalent combination of experience and training.

Responsibilities

  • Provides care management services to older adults and members of their families to improve their quality of life.
  • Serves as a liaison to other social service agencies, hospitals, clinics, courts, and community resources to assist clients in utilizing these resources to meet their needs.
  • Consults with professional staff in other program areas to discuss diagnoses and plans of treatment.
  • Assists older adults in managing income and getting maximum benefits from Social Security and Medicare; keeps clients informed of legislation affecting benefits.
  • Interprets program policies and procedures for individuals, groups, and agencies.
  • Prepares social studies, case histories, reports, and records.
  • Performs the full range of supervisory duties.
  • Employees in this job may participate in the performance of subordinates’ work consistent with operational or organizational requirements.
  • Performs related work as required.
