About The Position

The Philadelphia Corporation for Aging (PCA) is seeking an Aging Care Manager I to conduct ongoing assessment, service planning, service arrangement, follow-up, and reassessment for disabled and/or chronically impaired older adult participants. The role involves evaluating the medical, social, and psychological needs of individuals to offer services that improve their quality of life. The Care Manager I acts as a participant advocate and gatekeeper of service provision, collaborating with other agencies to refer individuals for additional services and assisting them in utilizing available community resources. Work is performed in accordance with State and Agency mandates and is monitored by the Supervisor through regular check-ins and system reports.

Requirements

  • BA/BS/BSW or MA/MS/MSW including at least 12 college-level credit hours in sociology, social welfare, psychology, or gerontology.
  • Minimum one-year work experience in social work or care management in the fields of aging, behavioral health, or disabilities.

Responsibilities

  • Maintain high quality care management services by identifying and meeting the varied needs of participants.
  • Perform all care management functions including assessment, service plan development, service arrangement, follow-up, monitoring, and reassessment.
  • Complete Needs Assessment Tool (NAT) and participant care plan at required timeframes.
  • Enter Care Management Activity in the form of units for services provided.
  • Work with participants, their family, and/or caregiver to develop an individual care plan.
  • Identify and mobilize informal and formal resources to meet participants' needs.
  • Facilitate participant choice of providers.
  • Arrange for needed services and entitlements, working cooperatively with participants and service providers.
  • Provide assistance to participants in the MA Application Enrollment process.
  • Conduct home visits and telephone contacts to monitor adequacy of individual care plans.
  • Monitor participant satisfaction to ensure quality of services provided.
  • Complete all necessary forms and data entry for participants' records.
  • Participate in orientation and training, and attend regularly scheduled supervision and staff meetings.
  • Contribute to upholding a highly supportive team culture.
  • Perform other duties as assigned.
