In this Aging Care Management Supervisor 1 position with the Area Agency on Aging for Luzerne/Wyoming Counties you will supervise and support staff within our Care Management Unit. In this position you will perform a variety of responsibilities related to general supervision, administration for care management and the personal emergency response program, and medical supplies. Your general supervisory responsibilities will involve planning, organizing, and assigning all work to staff to ensure timely completion. This includes providing training and guidance; monitoring, reviewing, and approving paperwork and data entered into the agency database; and evaluating staff performance. You will also be responsible for making supervisory field visits and representing staff needs and concerns to ensure good working relationships within the agency. Administrative responsibilities for care management will involve making presentations to community groups regarding agency related services, serving on committees, and participating in other Aging network activities. Your work with the personal emergency response program will include liaising with emergency response service providers, arranging for the resolution of practical equipment concerns, and monitoring usage of services and equipment. For Medical supplies, you will serve as our liaison with suppliers, monitor usage of supplies, and handle consumer or program issues as they arise. Requirements

  • Three years of experience in public or private social work and a Bachelor's Degree; or
  • Any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
  • You must meet the PA residency requirement.
  • You must be able to perform essential job functions.
  • A conditional offer of employment will require submission of criminal history reports.
