About The Position

This is a Care Management Supervisor I position within the Care Management Unit of the Area Agency on Aging for Luzerne/Wyoming Counties. Employee in this position supervises a unit of Care Managers, and one Case Aide. Employee is responsible for the Personal Emergency Response and Medical Supplies Programs. Planning, organizing, and assigning all work to staff to ensure timely completion. Providing training and guidance on policies and procedures in a prompt and knowledgeable fashion, and providing information to staff on appropriate resources available to the elderly. Implementing procedures to conform with PDA program policies, including, but not limited to OPTIONS, Cost Sharing, SAMS/OMNIA, Family Caregiver Support, Domiciliary Care, and Care Management programs. Monitoring, reviewing, and approving all paperwork and data entered into the agency database by care managers and analyzing for completeness and adherence to established standards. Conducting regular case conferences with each care manager. Conducting supervisory field visits. Representing staff needs and concerns to administrative staff to ensure productive working relationships within the agency. Specifying training needs of staff within the unit and arranging for appropriate training programs. Making presentations to community groups regarding aging services. Requirements

  • Knowledge of social casework principles and methods
  • Knowledge of current social, economic, and health problems and community resources as related to older adults
  • Knowledge of individual and group behavior and ways of working effectively with older adults who have social, economic, emotional, and/or health problems
  • Knowledge of the basic principles and methods of program interpretation and community organization
  • Skill in the development of plans to address the needs of older adults
  • Ability to work effectively with older adults and aid them to grow in the constructive use of their potential in adjusting to their specific problems
  • Ability to evaluate work of subordinates and to provide technical assistance and guidance
  • Ability to orient and instruct subordinates on procedures, techniques, laws and regulations
  • Ability to establish and maintain effective working relationships with clients, other members of the staff, outside agencies and institutions, and the general public
  • Ability to clearly express ideas orally and in writing
  • Three years of experience in public or private social work and a Bachelor's Degree; or any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
  • Be a resident of Pennsylvania.
  • You must be able to perform essential job functions.
