About The Position
The County of Sonoma Human Services Department (HSD) seeks an experienced Social Worker who is ready to make a difference in the lives of our community's most vulnerable adults and residents! Starting salary up to $56.15/hour ($117,191/year) plus a competitive total compensation package. The Sonoma County Aging and Disability Resource unit utilizes a no wrong door service delivery model, providing people of all ages, income levels, and abilities with equitable access to the information, assistance, and support they need to live as independently as possible in their own homes. This Aging and Disability Resource Social Worker will provide information and assistance, options counseling, short-term service coordination, and care management services to diverse, complex, and medically fragile individuals of all ages to support their needs in the community.
