The Afterhours RN Care Manager provides high-quality, compassionate hospice care outside standard business hours, ensuring seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. This role is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. The Afterhours Care Manager RN serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role, involving both direct patient visits and administrative support after hours. The position offers a full-time salaried schedule with options for working specific weeknights and every other weekend, with weeknight shifts from 5pm-8am and weekend shifts from Friday 5pm to Monday 8am.
