At Mountain Valley, what we do is personal, for the patients and families we serve, and for those of us who choose this work. We care for individuals facing serious and terminal illness, supporting them through physical, emotional, social, and spiritual challenges. We also help our communities better understand end-of-life care. For those called to this work, there is nothing more meaningful. Are you looking for a nursing role without a full caseload and nonstop visits—but still want to provide meaningful, hands-on care during some of life’s most important moments? This afterhours position is on call from home, but it is not fully remote. You must be available to travel for in-person visits when patient needs arise—including urgent symptom management and end-of-life care. You’ll cover Monday through Thursday (5:00 PM – 8:00 AM), responding to patient and family needs as they arise. There is no assigned caseload or scheduled visits—care is driven by patient needs and can vary throughout the shift. In this role, you will provide phone triage and support, travel to patient homes when needed, and deliver care for symptom management, active decline, and at times, support patients and families at the end of life—including during and immediately after death. These are often the moments when your presence matters most. At Mountain Valley Hospice, we believe hospice is about living well. Our afterhours nurses provide calm, compassionate support to patients and families, especially during the most critical and vulnerable moments. If you’re looking for a role with a different pace, meaningful work, and the ability to be present when it matters most, we’d welcome the conversation.
