The Aftercare Worker at Net Community Care, supports families who have successfully exited formal case-management services through DHS/CUA. This role focuses on reintegration planning, resource connection, and ongoing consultation to reduce the likelihood of re-entry into the child welfare system. The Aftercare Worker collaborates with case management teams, children, biological families, and external partners to ensure families are equipped with the tools and support needed for long-term stability. Success in the Aftercare Worker role means being a reliable and empathetic support system for families transitioning out of formal child welfare services. On a daily basis, the Aftercare Worker builds trusting relationships, maintains proactive communication, and collaborates with internal and external partners to implement effective aftercare plans. Empowering families through teaching essential life skills and connecting them to community resources is central to the role. Accurate documentation and consistent follow-up are key, as is the ability to adapt to each family’s unique needs. Ultimately, success is measured by the family’s ability to remain safely and sustainably independent, without re-entering the child welfare system.
