About The Position

Talbott Recovery is currently looking for a fulltime Continuing Care Coordinator/Discharge Planner for our Main Campus on Hunter Road, Atlanta. Requirements

• Experience in alcohol/substance abuse/12-Step program
• Experience in Discharge Planning, Case Management or Continuing Care Management
• Must have a High School Diploma or Equivalency; Bachelor Degree strongly preferred

Responsibilities

• Responsible for the coordination of administrative and discharge planning for all Talbott Recovery patients
• Establishes and maintains referral relationships
• Contacting referral sources upon admission and confirms patient's needs/expectations regarding updates/information
• Coordinates all communication and information exchange with referral source
• Develops a comprehensive and patient-focused continuing care plan for assigned patients
• Timely and accurate input of contact information into established system
• Ability to access, assess and report on requested and/or needed information
