The After Office Hours Care Manager is a core member of the after hours mental health program, working closely with patient's behavioral health providers and staff. The Care Manager is responsible for supporting and coordinating the mental health care of patients, monitors and tracks treatment response for changes in clinical symptoms and treatment side effects or complications. Supports and encourages individuals and family members to increase knowledge about their mental and physical health conditions, engage in their care, and improve adherence to treatments. Provides education and available treatment options. Facilitates referrals for other needs and supports patient medication management including monitoring adherence and ability to access obtained medications. Assists with managing patient appointments and phone calls for mental health care team, in coordination with support staff. Provides support to patients and families where psychosocial barriers interfere with their ability to access or maintain recommended treatments for behavioral health concerns.

  • Bachelor's degree in Psychology or a related field and two years of healthcare-related experience.
  • Bachelor's degree in Nursing, with NYS RN licensure/registration (or eligibility) and two years of post-RN experience.
  • Master's degree in Social Work from a CSWE-accredited program, with LMSW, LCSW, or eligibility, and three years of relevant post-masters experience (preferably in a hospital and/or healthcare setting).
  • Experience in a behavioral healthcare setting working with a multidisciplinary team.
  • Demonstrated prior interactions with patients in a medical setting.
