About The Position

Hope Hospice, a trusted community resource since 1980, is a non-profit, community-led agency focused on providing quality care to patients and families. The organization prioritizes patient comfort and health over financial demands. This position offers competitive pay and a supportive work environment. Hope Hospice is seeking a per diem After-Hours Nurse to provide skilled nursing and hospice care. The schedule is Saturday & Sunday, from 8:00am to 5:00pm, based out of the Pleasanton, CA office, covering Alameda, Contra Costa & San Mateo Counties. Hope Hospice adheres to all CDPH vaccine requirements for healthcare personnel. Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Current license as a registered professional nurse in the State of California.

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Responsibilities

  • Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
  • Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
  • Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
  • Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.
