About The Position
The Hospice After Hours Nurse plays a critical role in providing compassionate care to patients and their families during after-hours situations. This position involves responding to calls from patients, families, caregivers, and referral sources, assessing their needs, and utilizing critical thinking to deliver appropriate care. The nurse will employ the nursing process, which includes assessment, planning, interventions, implementation, and evaluation, to ensure that patients receive the highest standard of care. Effective interaction with patients, their significant others, and interdisciplinary team members is essential, as is maintaining professional nursing standards and clinical competency. In this role, the Hospice RN will report directly to the Clinical Director and will be responsible for ongoing assessments of the physical, psychological, social, and spiritual needs of patients and their families. The nurse will initiate appropriate interventions or referrals based on these assessments. Accurate and timely documentation of patient and family services is crucial, as is the provision of bereavement counseling when necessary. The nurse will also be expected to demonstrate knowledge of Hospice Care policies and procedures, as well as relevant state and federal regulations. Additionally, the Hospice After Hours Nurse will engage in community education regarding the philosophy and concepts of Hospice Care, representing the organization at nursing functions and community events. The role requires administering medications and treatments as prescribed by attending physicians and participating in ongoing education and in-service programs to enhance professional development. Physical demands of the job include the ability to move and lift patients who may weigh between 10 to 250+ pounds, often in the privacy of their homes.
