About The Position

The Aftercare Coordinator plays a crucial role in the Youth Supportive and Rehabilitative Program at the Merced Juvenile Justice Complex, focusing on the successful reintegration of returning citizens into their communities. This position involves developing a network of community-based service providers, managing a caseload of participants, and ensuring they are linked with necessary resources upon release. The role emphasizes the application of evidence-based practices to support participants in their transition from institutional settings to community life.

Requirements

  • High school diploma or equivalent required.
  • Bachelor's degree in social services, criminal justice, or a related field preferred.
  • Four years of experience in a criminal justice, treatment, or social services setting.
  • Thorough understanding of evidence-based practices related to successful reentry.
  • Ability to read and interpret technical procedures and legal documents.
  • Strong interpersonal skills to handle difficult contacts effectively.
  • Proficient in using computers and relevant software.

  • Experience working with youth in a rehabilitative setting.
  • Knowledge of community resources for returning citizens.
  • Skills in conflict resolution and crisis management.

Responsibilities

  • Develop and maintain a network of community-based service providers and referral sources.
  • Ensure returning citizens are linked with resources in their communities upon release.
  • Manage a caseload of participants and administer program policies and procedures.
  • Facilitate psycho-educational and behavior modification groups.
  • Prepare and update service and aftercare plans for participants.
  • Monitor participant progress and conduct regular follow-up calls post-release.
  • Assist participants in completing necessary forms for aftercare services.
  • Coordinate with various agencies to meet participants' needs.
  • Maintain confidentiality of participant information and files.
  • Prepare reports on participant behavior and progress.
