About The Position
The Advanced Social Worker provides direct patient care under the guidance of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health. This role may also involve providing clinical and administrative supervision. The Advanced Social Worker is responsible for delivering individual, family, and group psychotherapy in both ambulatory and hospital outpatient settings. They will conduct psychosocial assessments, complete Preadmission Screening and Resident Review (PASRR), manage discharge planning, and perform post-discharge calls. Additionally, the role requires knowledge of involuntary processes such as guardianships, advanced directives, and power of attorney (POA). The Advanced Social Worker will administer patient health questionnaires (PHQ-9), the Columbia-Suicide Rating Scale (C-SSRS), and screenings for Social Determinants of Health. They will also offer community resource knowledge and collaboration, and participate in continued education and in-service training.
