This position provides direct primary care, family planning and STD services to clients in Columbia and Hamilton counties. This position reports directly to the Columbia County Community Health Nursing Director. Provides healthcare, first aid and immunizations and evaluates and examines patients. Orders, conducts, interprets and/or evaluates diagnostic tests to identify and assess patient’s conditions. Refers patients for further study or services as appropriate. Consults with and discusses cases with Medical Director. Provides back-up in Hamilton County Health Department in the absence of the Medical Director. Responsible for family planning program management and compliance for both Columbia and Hamilton CHD’s. Completes monthly and quarterly reports. Monitors and completes all staff family planning training and reports. Prepare and schedule annual program compliance desk reviews and audits. Completes quality assurance and quality improvement for family planning program. Completes family panning sterilization reports. Maintains training and performs procedures for Women’s Health such as but not limited to IUD/IUS placement and removals. Responsible for follow-up of abnormal results for procedures and testing performed. Monitors the quality of care or standards of practice to ensure compliance with state and federal requirements. Participates with nursing staff in multi-county on call. Other related duties as required.
