Our Advanced Practice Registered Nurses (APRN) serve our community through providing direct clinical care of clients, including ordering of lab tests, counseling of patients, developing treatment plans and prescribing medication (within the scope of their education and certification of the collaborative agreement with their collaborating physician).   • Works with staff, as a team, to provide high quality care to our patients by identifying and assessing health concerns, examines the patient based on the health concern, and creating a plan specific to the patient needs.
  • Develops and maintains an awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.
  • Provides clinical consultation to other staff members and supervision of staff and students as assigned.
  • Demonstrates adherence to National Patient Safety Goals and plan.
  • Participates in quality improvement activities to assess, monitor and improve program services as well as mandatory in-services and drills, attends staff meetings and other training, including appropriate age specific educational trainings offered on a regular basis for professional development.
  • Graduate of Master's Degree or higher in Nursing and national certification as Nurse Practitioner: ANCC, AANP, AMCB, NCCC, NAPNAP.
  • Current Illinois licensure as a Registered Nurse.
  • Current Illinois licensure as Advanced Practice Registered Nurse or Advanced Practice Registered Nurse - Full Practice Authority.
  • Will apply for APRN-FPA within 6 months of becoming eligible based on state requirements.
  • State of Illinois Controlled Substance License.
  • Ability to maintain and keep records in EHR system.
