Responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. The position involves the direct provision of primary care medical services meeting all established productivity expectations, including examination and review of appropriate laboratory, referral, and imaging results to determine a diagnosis. The role requires accurate documentation of visits utilizing appropriate templates and recording relevant data to attain a diagnosis. The provider is accountable for attaining budgeted visits, meeting clinical care measures, and locking records in the 72-hour window. The position also involves providing comprehensive, continuous, and coordinated medical care for acute and chronic illness at all ages within the context of the family unit in a community setting, including health promotion, maintenance, and prevention of illness and disability. The provider will manage chronic diseases and acute illness, advocate for access to healthcare, consult with supervising physicians, and promote the Patient Centered Health Home model of care. Excellent customer service and care are essential, along with participation in daily huddles and collaboration with ancillary health team members.
