Responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. The position involves the direct provision of primary care medical services meeting all established productivity expectations, including examination and review of appropriate laboratory, referral, and imaging results to determine a diagnosis. The provider is accountable for attaining budgeted visits, meeting clinical care measures, and locking records in the 72-hour window. The position also involves providing comprehensive, continuous, and coordinated medical care for acute and chronic illness at all ages within the context of the family unit in a community setting, including health promotion, maintenance, and prevention of illness and disability. The provider will manage chronic diseases and acute illness, advocate for access to healthcare, consult with supervising physicians, and promote the Patient Centered Health Home model of care. Excellent customer service and care are essential, along with participation in daily huddles and collaboration with ancillary health team members.

  • Extensive formal training as a Nurse Practitioner or Physician Assistant.
  • Master's Degree or equivalent extended training program.
  • Licensed in the state of California as a Nurse Practitioner and/or Physician Assistant.
  • Experience in a Patient Centered Health Home model.
  • Strong communication skills for presentations and information synthesis.
  • Ability to create complex documents in Microsoft Word.
  • Advanced skills in Microsoft Excel for database management.
  • Ability to develop presentations in Microsoft PowerPoint.
