About The Position

We are hiring an APP for our Palliative team. This Palliative care Advanced Practice Provider (APP) position is a full-time role located primarily at: St. Luke’s Plaza with additional locations at SLE, SLS and SLN. The primary patient population is acute hospital admissions in need for Palliative supportive services. This is a Monday through Friday day shift position. Work hours are approximately 8 to 4:30. There is a call, weekend and holiday commitment for this role. Comprehensive knowledge base of common Internal Medicine disease processes seen in the acute hospital environment. Demonstrates basic science and up to date clinical knowledge Independently round on assigned patient panel, with direct collaboration with physicians. Communicates effectively with other team members and patients. Establishes a patient relationship characterized by mutual respect, empathy, and collaboration Gather objective and subjective data to develop differential diagnosis, assessment, and a plan of care. Perform accurate physical assessment. Interpret diagnostic and radiologic studies, labs, and review consultative recommendations. Place appropriate orders. Utilizes prescriptive authority in accordance with state and federal laws and regulations Performs ongoing assessment of patient response to plan of care, updates plan of care as needed based on response. Accurately documents consult and progress notes in timely manner, with appropriate billing. Communicates effectively using appropriate terminology, format, and technology in accordance health system policies Takes initiative for seeking opportunities to advance knowledge and experiences. Flexibility with scheduled rotations/locations as needed to ensure patient needs are met. Able to respond and adapt to rapidly changing patient conditions. Displays positive leadership skills amongst the multi-disciplinary team. Acquires new knowledge quickly and efficiently. Displays effective and clear communication amongst the multi-disciplinary team, including close collaboration with physician, consultants and nursing team. Minimum of 2-3 years of Palliative experience strongly preferred.

Requirements

  • Comprehensive knowledge base of common Internal Medicine disease processes seen in the acute hospital environment.
  • Demonstrates basic science and up to date clinical knowledge
  • Applicable Experience: Less than 1 year Advanced Practice Provider (KS) - Various, Advanced Practice Provider (MO) - Various
  • Master's Degree

Nice To Haves

  • Minimum of 2-3 years of Palliative experience strongly preferred.
