About The Position
The Advanced Practice Provider (APP) will serve as the clinical coordinator for identification, tracking and treatment alternatives for structural heart/valve patients, recommend diagnostic studies and/or clinical interventions when appropriate, as well as measure patient-specific and aggregate outcomes.
Responsibilities
- Coordinate patient care and evaluation inpatient and outpatient
- Design and align resources to support program initiatives, program development
- Develop and maintain relationships with internal and external referring physicians
- Develop program database to include but not limited to following procedures:
- Transcatheter Aortic Valve Replacement (TAVR)
- Percutaneous Left Atrial Appendage Closure (LAAC)
- Transcatheter Mitral Valve Repair/Replacement (TMVR) and other structural heart procedures
