About The Position

About the Role: The part-time Advanced Practice Provider (APP) at plays a crucial role in delivering high-quality healthcare services to our diverse patient population. Requirements

  • Master's degree in Nursing or a related field with a focus on advanced practice.
  • Current certification as a Nurse Practitioner or Physician Assistant.
  • Valid state licensure.
  • Advanced clinical assessment and diagnostic abilities, which are essential for accurately identifying patient needs and developing effective treatment plans.
  • Strong communication skills are vital for educating patients and collaborating with the healthcare team, ensuring that everyone is aligned in providing optimal care.
  • Problem-solving skills are utilized daily to address complex health issues and adapt care strategies as needed.
  • Organizational skills are important for managing patient caseloads and maintaining accurate documentation.

Nice To Haves

  • Experience in a primary care, healthcare for the homeless, or other community health setting.
  • Bilingual proficiency in English and another language relevant to the patient population.
  • Familiarity with eClinicalWorks (eCW) EMR.
  • Preferred skills, such as bilingual communication, enhance the ability to connect with a broader range of patients, fostering a more inclusive healthcare environment.

Responsibilities

  • Conduct thorough patient assessments, including physical examinations and health history evaluations.
  • Diagnose and manage acute and chronic health conditions, prescribing medications and treatments as necessary.
  • Collaborate with physicians, nurses, and other healthcare professionals to create and implement individualized care plans.
  • Educate patients and their families about health management, preventive care, and lifestyle modifications.
  • Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines.
