About The Position

We are seeking a dynamic and compassionate Advanced Practice Provider (APP) to join our high-performing Trauma and Emergency General Surgery team. This full-time role is a critical addition to support our efforts in improving continuity of care and reducing hospital readmissions. This APP will work closely with our trauma surgeons, general surgery providers, and multidisciplinary care teams to enhance both inpatient and outpatient care coordination, with a strong focus on transitional care and wound management.

Requirements

  • Minimum of 1 year clinical experience preferred, not considering new grads at this time
  • Masters Degree Nurse Practitioner Required or Bachelors Degree Physician Assistant Required
  • Certified Registered Nurse Practitioner Upon Hire Required or Licensed Physician Assistant Upon Hire Required
  • Basic Life Support Upon Hire Required or Advanced Cardiac Life Support Upon Hire Required
  • BLS and/or ACLS as appropriate for the clinical role. Upon Hire Required
  • Nurse Practitioner - board certified by American Nurses Credentialing Center Commission on Certification Upon Hire Required
  • Nurse Practitioner - Completion of an accredited program for nurse practitioners Upon Hire Required or Physician Assistant - Graduate of accredited physician assistant program Upon Hire Required

Responsibilities

  • Full-time position working Monday - Friday; no nights/weekends/holidays/call
  • Outpatient clinical support seeing and evaluating patients recently discharged from the hospital after having a traumatic injury or an emergency surgery consultation or admission
  • Provide ongoing care for trauma and emergency surgery patients in clinic who may need further workup or imaging to manage complications or complex disease
  • Conduct post-discharge follow-ups within 48 hours (via phone or in-person) to identify complications early and prevent ER visits
  • Manage and monitor wound care, dressing changes, and wound vacs in collaboration with the team
