About The Position

The Transplant Coordinator/Nurse Practitioner delivers professional, direct patient care, including problem evaluation and management, under the direction of a supervising medical doctor. Primary Age Groups Served (Patient Care Providers): School Age Child (5 – 12 years, Adolescent (13 – 17 years), Adult (18 – 64 years), Older Adult (65+ years) The position requires successful completion of an Accredited Nurse Practitioner program, or an Accredited Physician Assistant program, national certification through an approved agency and registration with New York State. Licensure in NYS is mandatory. Related work experience, educational background, work stability and interpersonal/communication skills are weighed heavily. Excellent verbal and written communication skills. Ability to effectively present information and respond to questions from physicians, patients and their family members or other employees within the work setting. Conducts preoperative teaching for transplant program patients and their families. Plans, implements, coordinates and evaluates clinical practice according to the Nursing Practice Guidelines of the Albany Medical Center. Acts as a consult and preceptor in the clinical practice. Evaluates current nursing practice and develops and implements new policies, procedures and standards of care as related to the Abdominal Organ Transplant Program. Ensures compliance with all United Network of Organ Sharing (UNOS) regulations Assists the attending Transplant surgeons, house staff, and nursing staff with the direct care of hospitalized patients and outpatients on a rotating basis with the other clinical care coordinators, including: Attends twice weekly outpatient Transplant clinics Performs initial history and physical on transplant patients Demonstrates appropriate judgment in prioritizing and recording patient problems and findings to the physician, and will consistently follow-through with implementation of the physician’s orders and plan of care Coordinates evaluation of prospective abdominal organ transplant recipients and living donors, participates independently with the health care team, and maintains waiting list of recipients with UNOS Monitor the current Wait list patients and identify barriers to being active and facilitate activating the recipient for both Kidneys and Pancreas Assists with data collection for research and/or program improvements for clinical practice Performs minor wound care as directed by providers Removes tubes and drains from surgical wounds Removes sutures, staples, and clips from surgical incisions Writes patient assessment and procedure notes in the progress notes section of the hospital and outpatient charts Schedule appropriate non-invasive laboratory tests and participates in the interpretation of the results and the institution of appropriate therapy Schedules appropriate surgery and tests for patients Discharge Planning Reviews discharge instructions to hospitalized patients Works with hospital case manager and social worker regarding patient discharge needs Answers telephone inquiries from patients and/or visiting nurses after hospital discharge Education of health care workers and the public regarding organ transplantation; participate in monthly pre-transplant educational seminars Additional responsibilities as needed. May be subject to infectious materials/blood-borne pathogens. Will have exposure to transient, indigent, institutionalized and incarcerated patients.
