About The Position

The Advanced Practice Provider (APP) at Prevea Health is responsible for delivering primary health care services to patients, families, and groups, with a focus on health promotion and disease prevention. This role involves collaboration with physicians and other health professionals in various aspects of clinical practice, education, and program development.

Requirements

  • Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program.
  • Willingness to learn new roles within scope of practice.
  • Demonstrates ability to develop and maintain effective working relationships.
  • Demonstrate organizational skills that allow timely patient appointments and appropriate required follow-up.

Responsibilities

  • Performs comprehensive health assessments for acute and chronic patients.
  • Assesses and manages care for acute, minor, episodic illnesses and injuries.
  • Interviews and advises patients/families regarding health/illness prevention.
  • Performs follow-up exams.
  • Provides assessment, evaluation, and management of clients with acute and chronic illnesses.
  • Collaborates with patients to determine physiologic, emotional, and environmental needs and priorities.
  • Provides case management services to assist clients and families with the arrangement for, or referral to, needed services.
  • Manages patient health concerns through telephone triage and consultation as needed.
  • Orders appropriate diagnostic and laboratory tests, prescribes pharmacological agents, and other non-pharmacological treatment modalities.
  • Maintains and reviews patient records, charts, and other pertinent information.
  • Performs patient callbacks, reports diagnostic findings, and provides consultation.
  • Participates with physicians in the review of patients' health records.
  • Participates in call coverage when required.
  • Attends required department meetings and participates in committees as requested.
  • Promotes ongoing quality improvement activities.
  • Documents in the electronic medical record for all patients.
