About The Position

Sentara Medical Group is seeking a Nurse Practitioner or Physician Assistant to join its Outpatient Palliative Care team at the Supportive Care Cancer Center located on the campus of Sentara Norfolk General Hospital in Norfolk, VA. The Nurse Practitioner or Physician Assistant will provide clinical care, education, and compassionate support for patients with advanced or serious illnesses as part of a collaborative interdisciplinary team. Position Highlights: Monday-Friday; 8:00 a.m. - 4:30 p.m. Performing outpatient palliative care consultations and follow-up visits for patients with complex symptom management and advanced care needs Facilitating family meetings focused on medical decision-making, advance care planning, and goals of care discussions Collaborating closely with referring oncology providers, social workers, and other interdisciplinary team members to ensure coordinated, holistic, and patient-centered care Supporting seamless transitions of care, including coordination with home-based palliative care or hospice programs as appropriate Participating in team meetings to promote best practices in symptom management, emotional support, and quality of life improvement for patients and families

Responsibilities
