About The Position
The Advanced Practice Provider in Primary Care at Summa Health Medical Group is responsible for developing and advancing the quality of advanced practice through specialized direct patient care, leadership, consultation, education, and research. This role focuses on improving processes of care for patients and ensuring quality of patient care by utilizing established standards of care for the specified department. The position involves coordination of services with physicians, care managers, social workers, patient care coordinators, therapists, and other community agencies as appropriate.
