About The Position

The Advanced Practice Provider – Rheumatology will work in an Outpatient Clinic within a Non-Profit Critical Access Hospital in Blackfoot, ID. The schedule is Monday – Friday, standard daytime clinic hours. The APP will work with adult patients with chronic and complex rheumatologic conditions. The APP will have dedicated Rheumatology physician support and experienced nursing staff and on-site Infusion Therapy Center team. The APP reports to Rheumatology Physician and Clinical Leadership. The reason for the opening is practice growth and increased patient demand.

Requirements

• Certified Nurse Practitioner or Physician Assistant
• Active Idaho license or eligibility to obtain
• BLS required
• Prior Rheumatology experience required
• Ability to manage an independent outpatient clinic panel and collaborate in a team-based environment

Responsibilities

• Diagnose and manage chronic rheumatologic conditions in an outpatient setting
• Coordinate care with on-site Infusion Therapy Center
• Collaborate closely with Rheumatology physician and multidisciplinary care team
• Provide comprehensive follow-up care and patient education
• Maintain accurate and timely clinical documentation
