About The Position

St. Catherine of Siena Nursing and Rehabilitation and St. Catherine of Siena Hospital are proud members of Catholic Health and have served the residents of the Smithtown area for more than 50 years. St. Catherine's is located on the north shore of Long Island in Smithtown. The campus is comprised of a 240-bed nursing and rehabilitation care center; 296-bed not-for-profit community hospital and a 60,000 square foot medical office building. Requirements

Graduate of a Masters in NP program
Nurse Practitioner License
NP Board Certification
BLS and ACLS certification

Nice To Haves

Prior experience in acute, sub-acute, rehabilitation and long term care preferred.

Responsibilities

Coordinates with the Medical Director, Attending Physicians, Nursing Staff, Rehabilitation Staff and Ancillary Support Staff in providing the direct and collaborative treatment plan for sub-acute, rehabilitation and long-term care residents.
In collaboration with the Medical Director and Attending Physicians, performs physical examination, comprehensive patient assessment, medical diagnosis and prescribes appropriate treatment modalities for this resident population.
Ensures that all tests, therapies, medications and treatments are initiated, followed and evaluated timely.
In collaboration with the Director of Nursing, monitors that the Nursing Home's policies & procedures and nursing standards of care are appropriately followed and maintained. Reviews/audits nursing assessment, planning, intervention, evaluation and medical record documentation for regulatory compliance.
Provides resident, family and staff education and participates in Performance Improvement.
Demonstrates care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights.
