About The Position

Franciscan Physician Network is seeking a full-time Nurse Practitioner or Physician Assistant with Palliative Care experience for an inpatient position to join our team at Franciscan Health Crown Point to meet the growing demands for Hospice and Palliative Care services. Ideal candidate will have past Palliative or Home Health Care experience, with a focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with our hospitalist team of physicians to coordinate and deliver the highest quality of patient care. Hospital based clinic hours: Monday – Friday, 8:00 a.m. – 5:00 p.m. Flexible full-time schedule; no holidays or weekends Average inpatient volume: 12 to 15 patients per day Inpatient follow-ups and new consultations Will clarify care planning with patient’s family Certification as a Hospice and Palliative Nurse (ACHPN®) or Certificate of Added Qualifications in Palliative Medicine and Hospice Care for PAs is preferred Eligibility to become certified within 2 years is required Franciscan Health is a non-profit health care ministry with primary and specialty care physician groups located throughout Indiana and Illinois. Requirements

  • Master's Degree - Required
  • Physician Assistant Certified (PA-C) - Required - or Nurse Practitioner (NP) or Nationally Recognized Certification - Required
  • Basic Life Support Program (BLS) - American Heart Association - Required
  • Eligibility to become certified within 2 years is required

Nice To Haves

  • Certification as a Hospice and Palliative Nurse (ACHPN®) or Certificate of Added Qualifications in Palliative Medicine and Hospice Care for PAs is preferred
