About The Position

Palliative Care Advanced Practice Provider Albany Medical Center Albany Medical Center is seeking a compassionate and skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Palliative Care team. This is an opportunity to provide holistic, patient- and family-centered care in the Capital Region’s only academic medical center as part of our adult consult service. About the Role: As part of our interdisciplinary palliative care team, the APP will provide specialized medical care for patients living with serious or life-limiting illnesses. The focus is on symptom management, quality of life, and support for patients and their families through all stages of illness, including end-of-life care.

Requirements

  • Current NYS licensure as a Nurse Practitioner or Physician Assistant (or eligibility)
  • National board certification as an NP or PA
  • Strong communication skills, empathy, and ability to work collaboratively within a multidisciplinary team
  • Commitment to patient- and family-centered care

Nice To Haves

  • Experience in palliative care, oncology, geriatrics, or critical care preferred

Responsibilities

  • Conduct comprehensive assessments of patients with complex medical needs, including pain, symptom burden, psychosocial concerns, and spiritual care needs.
  • Develop, implement, and adjust individualized care plans in collaboration with attending physicians, nurses, social workers, and chaplains.
  • Provide expert symptom management for issues such as pain, dyspnea, nausea, anxiety, and delirium.
  • Facilitate advance care planning discussions and assist patients and families with understanding goals of care and treatment options.
  • Participate in family meetings to ensure alignment between medical recommendations and patient preferences.
  • Provide palliative care consultations for hospitalized patients and assist with transitions of care to home, hospice, or other settings.
  • Educate patients, families, and healthcare team members on palliative care principles.
  • Contribute to quality improvement initiatives and participate in palliative care education for trainees.
