About The Position

The Advanced Practice Provider in Palliative Care/Hospice at Monument Health is responsible for managing complex patients and collaborating with healthcare teams under physician supervision. Requirements

• Certified Nurse Practitioner (CNP) certification in South Dakota and Wyoming.
• 1+ years of clinical experience preferred.
• 1+ years of healthcare experience preferred.
• Ability to perform light work, exerting up to 20 pounds of force occasionally.

Responsibilities

• Assume primary responsibility in specialty area under physician supervision.
• Manage selected at-risk or complex patients in collaboration with healthcare team members.
• Spend more than 20% of time in both outpatient and inpatient settings.
• Assess, diagnose, treat, and manage patients' healthcare problems/needs.
• Apply clinical interventions, evaluate outcomes, and implement changes to the plan of care.
• Communicate changes in patient status to meet patient needs.
• Share in practice call requirements as needed.
• Perform procedures within the practice scope, excluding operating room or first assist duties.
• Complete thorough and timely documentation in accordance with facility policy.
• Comply with scope of care under federal, state, and local regulations.
• Demonstrate knowledge of disease processes and conduct comprehensive health assessments.
• Order appropriate tests, procedures, and therapies in collaboration with medical staff.
• Recognize physical, developmental, and psychosocial distress of patients and provide interventions.
• Respond appropriately to emergency situations.
