UW Health Northern Illinois Center is looking for an experienced Advance Practice Provider to join the Medical Oncology team at the Carbone Cancer Center in Rockford, IL . Our philosophy is to provide compassionate, supportive, comprehensive cancer care for our community. The Advance Practice Provider is responsible for providing comprehensive care to both well and ill patients in an outpatient setting, working collaboratively with our team of Medical Oncologists, PA and APRN. He/she demonstrates a high degree of clinical expertise in working with patients with acute and chronic illnesses, he/she will also provide care and educate patients and their families on wellness care and treatment plans for health disorders and intervenes in acute episodes. He/she is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients and families with acute and chronic health needs. Your clinical support includes the following : Clinical Duties Conduct comprehensive patient assessments, including history and physical exams. Diagnose and manage oncologic conditions in collaboration with oncologists. Develop and implement treatment plans, including chemotherapy, immunotherapy, targeted therapy, and supportive care. Order and interpret diagnostic tests such as blood work, imaging, and biopsies. Manage treatment-related side effects, including nausea, pain, fatigue, and immunosuppressive complications. Monitor patients for disease progression and response to treatment. Patient Education & Support Educate patients and families about diagnoses, treatment options, side effects, and supportive care measures. Guide patients on symptom management and lifestyle modifications. Provide emotional support and coordinate referrals for mental health or social work services. Collaboration & Coordination Work alongside oncologists, radiologists, nurses, pharmacists, and other healthcare professionals in a multidisciplinary setting. Coordinate patient care, including referrals to radiation oncology, surgery, or specialty services. Administrative & Leadership Roles Document patient encounters and treatment plans in electronic medical records (EMR). Ensure compliance with hospital policies, treatment protocols, and regulatory guidelines. Engage in quality improvement initiatives and contribute to institutional cancer programs. Provide mentorship and training to nursing staff or student trainees.
